Von Willebrand factor von Willebrand factor multimers within-subject variation bleeding disorders

Objective: Accurate interpretation of von Willebrand factor multimer (VWF:MM) assays is critical for the diagnosis and classification of von Willebrand disease (VWD). To date, no published data have addressed within-subject variability using the semi-automated Hydragel VWF:MM assay.

Materials and Methods: This study evaluated within-subject variation in VWF:MM patterns among 26 individuals assessed for bleeding disorders. VWF:MM analysis was repeated over time using Hydragel H5VW and H11VW gels (Sebia, France).

Results: Across all patients, repeated VWF:MM assessments demonstrated consistent interpretations. Two exceptions occurred due to sample collection after infusion with factor concentrates. These cases were excluded from the final analysis. The study highlights the importance of treatment history in multimer interpretation.

Conclusion: This is the first report to demonstrate reliable within-subject consistency using the semi-automated Hydragel VWF:MM assay. The findings support its clinical utility for VWD diagnosis and monitoring, provided that timing of sample collection relative to factor treatment is appropriately considered.
