Simultaneous surgery laparoscopic cholecystectomy pelvic pathology single optical port

The technique of simultaneous laparoscopic operations for gallstone disease combined with benign pelvic pathology in women was developed and analysed. Using a single umbilical optical port for both the cholecystectomy and gynaecological stages, combined with three to four 5-mm working ports, reduced the number of abdominal-wall incisions from 7–10 (with two separate laparoscopies) to 5–6. Table tilt between stages was achieved by rotation without patient repositioning. The stage sequence was determined by cholecystitis severity and the expected gynaecological-stage duration: cholecystectomy first as a rule, cholecystectomy first for acute inflammation, and the gynaecological stage first when its expected duration exceeded 90 minutes in chronic calculous cholecystitis. The laparoscopic approach reduced blood loss, pain and hospital stay and lowered the total cost of treatment by 64 % compared with staged management.
